New Patients

Please ensure you have all the Documents listed here before you start to register online.  Any incomplete forms or missing documents – your application to register may be REJECTED. Thank you.

Documents Needed:

Photograph ID – Passport / European ID Card / Full Driving License

Proof of Address – Current Utility Bill / Tenancy Agreement / Bank Statement

Children – Passport or Birth Certificate AND Child Immunisaton Records

When you register with us you will be given a registration form and an appointment for a New Patient’s Health Check. Please be prepared for the registration by bringing your medical card, your personal identification ( original birth certificate or passport) and proof of address document (Current Utility bill or band Statement) . Registration starts after 10am.

All new patients including children will be asked to see the nurse. If you are registering a child please give us copies of immunisations and if you are a woman over the age of 25y please bring a copy of your last smear report. If you are on medication please bring the list of medicines and doses.

Online ‘Pre-Registration’ With The Practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the “Send” button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
